Out of Network (OON) Insurance - We work with insurance companies and will attempt to set up one time agreements for in-network billing so that you can get services using your insurance based on in-network benefits.

Or, we provide superbills (detailed receipt for insurance) can be provided - please check with your insurance for OON benefits and reimbursement. This means you will pay for each appointment out of pocket and submit a monthly invoice (superbill) to your insurance for them to reimburse you.

Private pay accepted - Both credit/debit card and HSA/FSA payments can be used to pay for services. If you will not be using insurance, under the No Surprises Act, you will be provided with a good faith estimate in writing that outlines expected costs as well as the option to complete the sliding fee application.

Sliding Fee Scale Program - It is the policy of Halcyon Counseling and Consulting, LLC to provide essential services regardless of the patient’s ability to pay. Halcyon Counseling and Consulting, LLC offers discounts based on family size and annual income.

The discount will apply to all services received at this clinic, but not those services or equipment purchased from outside, including reference laboratory testing, drugs, and other such services. You must complete this form every 12 months or if your financial situation changes.
